SmartForms are now available for research data capture within the Research Studies activity. The SmartForms are custom built by IT per study. An eServices ticket may be placed to begin the process of development and build. 

Improve study accrual and increase recruitment efficiency by using MyChart to find out whether potentially eligible patients are interested in participating in a study. Positive patient responses route to the research coordinator's In Basket for follow up. Enter E-services ticket for this feature. Additionally, researchers can now see four new enrollment statuses: Interested, Not Eligible, Declines and Messaged.

MyChart Questionnaires filled out by the patient prior to an office visit now appear in Chart Review. Go to Encounters tab > Office Visit > Detailed Report. The answers will be at the bottom of the detailed report along with historical data columns for previously answered questionnaires.
